Application Process:

1. Select the application form above that is specific to your business type. For all other business types, complete the “General Application”.
2. Download and rename the application with your business name and date.
3. Fill out the application form.
   1. Type your responses into the writable PDF form, OR
   2. Print a copy of the application and complete.
4. Send the completed application using one of the following delivery methods:

5. Your application will be reviewed as soon as it is received. You will receive a confirmation call to schedule a Pollution Prevention Assistance visit.
